What is Thyroid Function Test?

Thyroid Function Test is a group of assessments that are carried out together to identify or diagnose the thyroid diseases. It measures the stages of following three hormones in the blood: Thyroid Stimulating Hormone (TSH), Thyroxine (T4) and TriIodothyronine (T3).


Why is Thyroid Function test done?

To diagnose any suspected thyroid ailment (Hypothyroidism or Hyperthyroidism)

To observe the patient’s response to thyroid illnesses treatment

To inspect the reason of infertility


What does Thyroid Function Test Measure?

The Thyroid Test measures the tiers of the following three hormones in the blood:

Thyroid Stimulating Hormone (TSH)

Thyroxine (T4) - Free

TriIodothyronine (T3) - Free

The thyroid gland (a small, butterfly-shaped gland found in front of the neck) produces the following hormones:

Triiodothyronine (T3)

Thyroxine (T4)

Thyroid Stimulating Hormone (TSH) is a hormone secreted into the blood through Pituitary gland (a gland present in the brain). It directs thyroid gland to produce and discharge the thyroid hormones (T3 & T4) into the blood. The iodine from the food arouses the thyroid gland to make the thyroid hormones.

The thyroid hormones regulate the development and metabolism of the body. If the thyroid gland produces very excessive quantities of these hormones (T3 and T4), you might also notice signs of weight loss, speedy heartbeat, tremors, sweating, anxiety, over sensitivity toward heat, etc. and this is recognized as Hyperthyroidism.

The reductionin the buildup of thyroid hormones (T3 and T4) results in Hypothyroidism which may lead to weight gain, fatigue, slow heart rate, more sensitivity to cold, depression, dry and thin hair, etc.

There is a report system machine in the body to secure amounts of the thyroid hormones (T3 and T4) in the blood. When the levels of thyroid hormones decrease, the pituitary gland is prompted to discharge TSH.

High TSH in subsequently boost the discharge of T3 and T4 hormones from the thyroid gland and vice-versa.

T3 and T4 flow into in the blood in two forms:

1) Bound form - It is bound to proteins present in blood and this stops it from getting into the body tissues. The three essential proteins in the blood that the thyroid hormones are tied to are albumin, transthyretin and Thyroxine-binding globulin (TBG), known as Thyroid hormone Binding Globulin (THBG).

2) Free form - It gets into the body tissues where it is required.

The whole T3 or complete T4 comprises the bound and the free types circulating within the blood. As a result, thyroid hormones can be measured as Free T3, Total T3, Free T4 and Total T4.
The Thyroid Test measures the types of T3 and T4 i.e. FT3 and FT4 alongside TSH.


INTERPRETING THYROID FUNCTION TEST RESULTS


Low T4 and T3 alongside a high TSH stage suggest hypothyroidism. The most common cause of hypothyroidism is Hashimoto thyroiditis

High T4 and T3 alongside a low TSH suggest hyperthyroidism. The most common cause of hyperthyroidism is Graves’ disease

Normal thyroxine T4 and T3 alongside excessive TSH generally suggests mild or subclinical hypothyroidism

Normal T3 and T4 alongside low TSH suggests mild or subclinical hyperthyroidism

Note: TSH levels show circadian disparity (fluctuates during the 24-hour cycle), reaching height levels between 2 - 4 am and are at low stages between 6-10 pm.
